In the manga, mahjong is a worldwide sport/game. In the anime, mahjong seems to be only a little more popular than it is today. In either case, it's not yet revealed whether riichi mahjong is the main variation of mahjong that is played outside of Japan.

Most of the in-universe media covering mahjong are unaware of the existence of mahjong related supernatural powers.
